Dear Sir / Ma'am,

This is to bring to your kind notice that I have a saving account in your bank for which an ATM card was also issued. The savings account number is XXXXXXXXX.

Yesterday while returning from office, my wallet got stolen and the ATM card was in it. I have launched an FIR for the same and a copy of which has been enclosed with this letter. I request you to kindly block the ATM card at the earliest to prevent any misuse of the same.

Also, I would be grateful if you could issue me a new ATM card at the earliest.
